Thyroxine, or T4, is the main hormone made by your thyroid gland. It helps control how quickly your body uses energy. Free T4 is the portion that is not attached to proteins in the blood and is available for your body to use. It is usually measured alongside TSH, and the two results together show whether the thyroid is underactive, overactive or working normally.
Why is this test done?
- To follow up an abnormal TSH result
- To investigate symptoms of an underactive or overactive thyroid
- To monitor thyroid treatment
- When a pituitary problem is suspected
What does a high free T4 mean?
A high free T4 usually means the thyroid is overactive (hyperthyroidism), especially if TSH is low.
Possible reasons include:
- An overactive thyroid, for example from Graves' disease or thyroid nodules
- Inflammation of the thyroid (thyroiditis), which can release stored hormone
- Too high a dose of levothyroxine, or taking it shortly before the test
- Some medicines, such as amiodarone
- Biotin supplements interfering with the test
What does a low free T4 mean?
A low free T4 usually means the thyroid is underactive (hypothyroidism), especially if TSH is high.
Possible reasons include:
- An underactive thyroid, often from Hashimoto's thyroiditis
- Too low a dose of thyroid medicine
- Serious illness
- Rarely, a problem with the pituitary gland, when TSH is not high as expected
Why we don't list a "normal range". Reference ranges differ between laboratories and can depend on your age, sex and whether you are pregnant. The range printed next to your result on your own report is the one that applies to you, so use that, and talk to your doctor about what your result means for you.
What can affect your free T4 result?
- Taking levothyroxine shortly before the blood test
- Biotin supplements
- Pregnancy
- Serious illness

- When should my thyroid be checked again?
Frequently asked questions
Why are TSH and free T4 tested together?
TSH shows how hard the brain is pushing the thyroid, and free T4 shows how much hormone the thyroid is making. Together they show where a problem lies.
What does free mean in free T4?
Most thyroxine in the blood is attached to carrier proteins. The free part is not attached and is available for your body to use, so it is a better guide to thyroid activity.
What is T3?
T3 is the more active thyroid hormone, mostly made from T4 in the body. It is sometimes tested if an overactive thyroid is suspected.
Can Patiently AI explain my free T4 result?
Yes. Paste your results, or upload a photo, PDF or Word file, and Patiently AI explains them in plain English, sticking to the numbers and ranges on your own report rather than adding its own, and suggests questions to ask your doctor.
